Our forms are provided above for you to fill out ahead of time but if you forgot them or don't have time, we also have copies you can fill out in-office.

A new patient appointment usually lasts 45-60 minutes so be prepared. However, that estimate includes the time it take to do intake paperwork so you do not have to show up early.

Walk in and our office manager will greet you and check you in. You will be directed to a touchscreen tablet where there are questions about your medical history, family history, surgical history, medications, and complaints. These are important for us to get a full clinical history so we can treat you in the best way for your body, so please fill them out as fully as possible.

Once you've finished on the tablet, you will either finish the forms that we have above, or get your vitals taken.

After vitals are finished, you are ready! Dr. Pirner will examine you and work out the best course of treatment to get you feeling 100%!
